As a general rule, it's only if you have a very old login form, haven't refreshed the page in 2 days before logging in...

Well, that's the Bad Behaviour mod and something is definitely screwy. The error code it gives you suggests you had a window open for a very long time before logging in.

Just restarted Chome and now is OK again.
