Adding custom action | Integration hooks

Hi folks,

I failed to make the custom hook work. For some odd reason, I'm only seeing the index page instead of my own hooks theme.

I followed the instruction from SMF official how to make a custom hook, which can be found here

I'm trying to make a simple page first with this custom hook, so I can start with more detail and advance customization

Along this post is everything I used to install this mod packed in RAR. I don't see any error message in installing, so I'm expecting that it has install successfully in my forum.
