I know I'm suggesting the obvious, here. But, well, sometimes we miss those things.

Database name correct? (I'm thinking uppercase/lowercase and ******).

Password correct? (Again, I'm thinking of case).

Does the database have a proper MySQL User installed, with all privileges?
